General Safety Guidelines for Production
Every production employee should receive Form BC1 with his or her start-up package, and should sign the accompanying Employee Acknowledgment.

Young and New Worker Orientation
Every young (under age 25) and new (new to the production or new to the location) worker is to be given a health and safety orientation. This form combines a checklist from actsafe with an attendance sheet. This form should also be used for health and safety orientation of extras and theatrical day hires.

Form 7: Employer's Report of Injury or Occupational Disease
The Workers Compensation Act requires you to submit this report witihn three days of an injury to an employee. Failure to do so may result in penalties.

NEW JANUARY 1, 2016: WorkSafeBC EMPLOYER INCIDENT INVESTIGATION REPORT (EIIR)
Use this form to document investigations of accidents, incidents and injuries. Should also be used to investigate "near miss" incidents. For instructions on completing this form, use this guide.

Emergency Plan Meeting
The First AD should give an Emergency Plan Meeting on the set for the entire cast and crew at show start-up, and all cast and crew members should sign the accompanying attendance sheet.

Serious Incident Reporting Procedures
Incidents that result in transportation by ambulance, visitation to the hospital by one or more employees, any treatments other than general first aid, or any serious property/asset damage, must be reported according to the procedures outlined on this form.

Before any stunt or pyro fx can be approved, the appropriate questionnaire below must be completed and submitted to the people listed at the top of the questionnaire.
